About Nathan W. Davis, DPM
Dr. Nathan W. Davis is board certified in foot surgery and has been trained specifically and extensively in the diagnosis and treatment of all manner of foot and ankle conditions. This training encompasses all of the intricately related systems and structures of the foot and lower leg including neurological, circulatory, skin, and the musculoskeletal system, which includes bones, joints, ligaments, tendons, muscles, and nerves.
A native of Bountiful Utah, Dr. Davis started his pre-medical education at BYU and finished at the University of Utah before attending medical school. He attended the California College of Podiatric Medicine in San Francisco, CA where he graduated with honors in 1998. After accepting and completing a three year surgical residency in the Detroit Michigan area, he opened up practice in the Murray, Utah area where he has remained for over 21 years. As a current resident of Murray, Dr. Davis has loved working and living with the people in the Salt Lake area and looks forward to many more years of practice here in the future.

Insurance:
We accept most insurance plans, Medicare and Medicaid, the few that we do not accept are listed below.
We will bill your insurance company, however, medical insurance is a contract between you and your insurance company and it is your responsibility to be knowledgeable of your insurance plan, i.e. referrals, co-pays, deductibles and other benefits. Please be advised that not all services are covered by all plans, specifically orthotics and other medical supply items.
For any surgical treatment, make sure you are aware of any restrictions, referrals, change in co-pay percentages, or pre-authorization that are required. Understanding and discussing this information with us early will help you have a smooth billing and payment experience.

Payment:
Office visit co-pays and non-covered services and products must be paid for at the time of service. Our office accepts cash, check and all major credit cards.
A cash discount will be given to those that have no insurance and payment must be made at the time services are rendered.
Cottonwood Podiatry is dedicated to providing you with the highest quality, cost effective medical care. We accept all major credit cards and offer payments installments. If your insurance doesn't cover your visit in full, payment plans are available for as low as $20 a month.
